If you would like to purchase a
copy of a Kentucky Health episode…
(1) Please mail a check or money
order payable to University of Louisville to the address below.
We do not accept credit or debit cards. (2) Let us know which
episode and media type you need. DVDs or VHS tapes are $15.00
each, plus $5.00 shipping per order. If no format is specified,
a VHS copy will be sent to you. Our programs are copyright protected
material and may not be copied, so you must order multiple copies
if needed. (3) Also remember to provide your shipping address
(not a Post Office Box) and your phone number.
Allow 2 to 4 weeks for delivery.
